The Skilled Nursing Facilities Program is available for individuals 18+ living skilled nursing facilities and requires a referral from the Department of Mental Health PASRR office.

Open Sky staff work as partners with you, your family, facility staff and medical providers to support your physical and behavioral health so that you live a healthy, fulfilling life.

Services provided include:

  • Goal setting — Identifying what is important to you and working toward it
  • Resource connections — Connecting you with behavioral health, community, and other helpful resources
  • Care coordination — Working with your loved ones and providers to support your wellbeing
  • Ongoing support — Checking in and providing continued support as your needs change
  • Interested in learning more about services? Please speak with your nursing facility social worker, who can help determine the appropriate program and referral pathway.
