At Open Sky, we understand that life’s challenges can make it difficult to access basic necessities like housing, food, and transportation. That’s why the Health-Related Social Needs (HRSN) Program is here—to provide immediate support for individuals facing barriers to essential resources.
We partner with MassHealth and several Accountable Care Organizations (ACOs), including Revere Medical, C3, and UMass/Tufts, to help individuals experiencing:

Unsafe or unhealthy living conditions

Food insecurity

How We Can Help
Community Supports Program for Homeless Individuals (CSPHI)
A navigator program providing one-on-one support for individuals of any age experiencing homelessness, with a focus on mental health and well-being.
Housing Search Assistance
Helping individuals 55 and older who are currently unhoused find stable, long-term housing.
Resources for Housing Transitions
Assistance with first and last month’s rent, security deposits, and furniture for individuals moving from homelessness into housing.
Housing Navigation
Guidance for individuals facing frequent emergency department visits and struggling with housing instability.
Healthy Homes Support
Assistance for individuals living in unsafe or unhealthy housing due to a disability or medical condition.
Nutrition & Kitchen Support
Nutrition vouchers and kitchen essentials for individuals with very low food security.
